Why people keep reordering it
- Active methylated forms
- B vitamins in the forms your body can put to use right away.
- Mood and hormone balance
- Supports healthy methylation, mood and hormonal rhythm.
- Energy at the cellular level
- Feeds the pathways behind steady, real energy.
Best suited to: vegan or plant-based for years, over 60 and eating less meat, struggles to swallow tablets, wants a matched dose, not the biggest number.
What the research supports
One sublingual tablet gives 2,500 mcg of methylcobalamin, the coenzyme form of B12. Ninety tablets at one a day is three months.
Oral repletion trials use 1,000 to 2,000 mcg a day.
- Energy and focus (evidence: moderate, from randomized controlled trial)
- Correcting a real B12 shortfall restores energy and mental clarity in trials, while adding more B12 on top of an already normal level does nothing measurable.
- Nerve comfort (evidence: moderate, from randomized controlled trial)
- B12 is needed to build and maintain the fatty sheath around nerves, and a shortfall produces nerve signal changes that improve once B12 is topped back up.
- Heart health (evidence: strong, from randomized controlled trial, meta-analysis)
- B12 given with folate helps clear a heart-taxing blood marker called homocysteine, and the drop is biggest in people who were low in B12 to begin with.

Is it right for you
Worth it if
- Vegan or plant-based for years
- B12 occurs only in animal foods, and the passive absorption a 2,500 mcg dose leans on happens without stomach acid or intrinsic factor having to cooperate.
- Over 60 and eating less meat
- Stomach acid and intrinsic factor both fall away with age, and a dose this size is built so the passive fraction alone still delivers about ten times a daily need.
- Struggles to swallow tablets
- It dissolves on or under the tongue on a mannitol base, so nothing has to go down whole and there is no sugar in it.
- Wants a matched dose, not the biggest number
- 2,500 mcg sits just above the 1,000 to 2,000 mcg used in the repletion trials rather than four times over it.
Look elsewhere if
- Wants routine daily B12 cover
- A 250 to 1,000 mcg tablet covers ordinary daily needs for less money. Doses in the thousands are for filling a real gap, not for maintenance.
- Tired and not sure why
- Get your level checked first. B12 restores energy when your level is genuinely low and does precisely nothing when it is already fine, and three months of tablets will not tell you which of the two you were.
- Wants full B vitamin coverage
- A B-complex, which brings folate and B6 along. Those two work with B12 on the same methylation step and are the usual co-travellers when one of the three runs short.
What to expect
If your level really is low, energy and mental clarity usually improve over two to six weeks as stores refill and new red cells are built. If your level is already normal, expect nothing, because there is nothing to correct. The bottle runs three months and the label points you to a practitioner past six months, so two bottles is the natural run before rechecking.
How it compares
- Compared with a 1,000 mcg cyanocobalamin tablet
- Two and a half times the amount, in the coenzyme form cells use directly rather than one that has to be converted before it is any use. The tradeoff: Cyanocobalamin is cheaper, more stable on a shelf, and is the form used in most of the trials that proved oral B12 works at all. Methylcobalamin's edge is more theoretical than demonstrated.
- Compared with a 5,000 or 10,000 mcg sublingual
- Nothing you should pay for. 2,500 mcg already clears the amount used in the repletion research, and B12 is water soluble, so the surplus leaves in urine. The tradeoff: A larger tablet does give more headroom if your absorption is badly impaired, which is worth having if a practitioner has told you that is your situation.
